SERVICES FOR SENIOR ADULTS
ANNUAL WELLNESS VISITS
Annual Wellness Visits are completed at no charge to qualifying Medicare patients. They include:
Check of height, weight, blood pressure, and other routine measurements
Health risk assessment
Review of your functional ability and level of safety
Update your medical and family history
Update list of your current providers, durable medical equipment (DME) suppliers, and medications
Create a written 5-10 year screening schedule or checklist
Screen for cognitive impairment, including diseases such as Alzheimer’s and other forms of dementia
Depression screening
Health education and preventive counseling may relate to weight loss, physical activity, smoking cessation, fall prevention, nutrition, and more.
CHRONIC CARE MANAGEMENT
Chronic care management offers additional help managing chronic conditions like arthritis, asthma, diabetes, high blood pressure, heart disease, osteoporosis, mental health issues and other conditions.
This includes a comprehensive care plan that lists your health problems and goals, other health care providers, medications, community services you have and need, and other information about your health.

IN-HOUSE LAB TESTING
Aim Health Clinic is pleased to offer in-office blood draws for routine lab testing.
We also offer rapid upper respiratory and urinary tract infection testing for quick results.
PREVENTIVE CARE
We"ll help you keep up with your preventive screenings for cardiovascular risk, diabetes, certain cancers and depression.
We also offer preventive immunizations for things like flu, pneumonia, and shingles.
PATIENT EDUCATION
We're happy to offer additional programs and classes for patients dealing with a variety of different health concerns.
